Hello Partha,
Yes, there is a good possibility for you to secure admission with your profile as each university is independent and admission parameters and weightage is different in different universities. Therefore, it all depends on which university you wish to apply and also on which country. Do a good research or follow pre-application process.
Admission in Canda will be difficult as there are few English universities being a bi-lingual country with English and French population. Apply early to universities which will accept your profile is a golden rule for you. For example for Aug/Sept/Oct 2014 intake, send your applications and documents on or before 15th January 2014.
Further, you have "Statement of Purpose" (SOP) which is part of admission evaluation where you could give reason on your low academic performace (5 years). If you have shown improved perfomance in your last year you could state the same in your SOP to show you have improved and that you are quite focused on your career now. You need to further reason on why you have selected particular country, particlular university, what has the program/course for you, how will you contribute to the program/course, your strengths, your short term and long term career objectives. Do not make your SOP long (not over 550 words approx.) but give them a clear idea about yourself and your career goals. Strong SOP will help you improve your admission chance. Also strong reference letters from referees will add value to your profile and improve admission chances. All the best!
