Hi, A Conditional offer generally means that you have got an admission but with certain conditions. The conditions could be passing your Bachelors or taking exams like IELTS or TOEFL with minimum marks or getting minimum percentages in your UG, etc. If you are not able to fulfill the conditions, you will loose the admission.

By the way, where did you get the admission and for what course.
