Which college near Fridley, Minnesota provides the physical therapy course for coming 2013 spring season?

Dear Sir, It’s an honor to write a mail to you though I want to know about the admission for the physical therapy course in coming 2013 spring season. I am currently in India but I am coming to US Minnesota this year and I am going to take admission as fresher. So, what should I do about the admission process there once I reached there? I will bring my high school transcripts but I am confused that do I have to submit the TOEFL score and SAT/ACT score compulsorily? Please enlighten me on this problem.
